If you are in need of a new health care provider at Prevea, we want to make this transition easier for you. We have included information on this page about looking for a new provider, or you can call our Patient Loyalty Line at toll free 855-81-LOYAL. The patient loyalty line is handled by a group of caring individuals that have been training to help you find a new provider to fit your specific needs. Calls are taken from 7:30 a.m. to 5:30 p.m. Monday through Friday.
